PFN, PFRN and PFIN get used interchangeably in purchase orders across India — and that causes wrong deliveries, delayed surgeries and billing disputes. They are three distinct nail systems. The one-paragraph answer
PFN (Proximal Femoral Nail) is the classic intertrochanteric fracture nail using two proximal screws (a lag screw plus an anti-rotation hip pin). PFRN (Proximal Femoral Rotation-control Nail) is the newer generation using a helical blade or enhanced screw design for better rotational stability in osteoporotic bone. PFIN (Proximal Femoral Integrated Nail) integrates the proximal fixation into a combined mechanism for specific fracture patterns. If your surgeon says "PFN2" or "nail with blade", they usually mean a PFRN-class implant.

Sizing that purchase teams get wrong
- Diameter: 9 to 12 mm are the workhorse sizes. Confirm from the pre-op templating, not from "whatever came last time."
- Length: short nails (180–240 mm) for standard IT fractures; long nails (300–420 mm) for subtrochanteric extension or pathological fractures. A "PFRN nail Ø11 L300" is a long-nail case — make sure the long-nail set is booked, not the short set.
- Neck angle: 130° and 135° are both stocked; the surgeon's templating decides.
- Side matters on some systems: long nails have left/right bow — a TAN LEFT nail cannot go into a right femur.
Titanium vs stainless steel
Titanium nails dominate current practice: better biocompatibility, MRI-friendlier, and the standard for premium sets (Meril's titanium PFRN systems are what we dispatch most). Stainless remains available where budget is the deciding factor.
What a complete set dispatch includes
A proper proximal femoral nailing dispatch is not just the nail. Confirm the box contains: the nail (correct diameter, length, angle, side), the proximal screws or helical blades in a size run, distal locking bolts, end caps, and the instrumentation tray for that specific system — PFN instruments do not fit PFRN implants. Sets travel with a batch-tracked challan and return within the agreed loaner window.
